Nach einer Einfuhrung in das Thema enthalt dieses Buch Formulierungshilfen (mit Hinweisen aus den Richtlinien des MDK) und Vorlagen (z. B. Stammblatt, Pflegeanamnese, Pflegeassessment, Biografie etc.). Ausserdem werden die ambulanten und stationaren Pflegetransparenzkriterien mit den entsprechend erforderlichen Dokumentationsunterlagen aufgelistet. Der Anhang enthalt u. a. einen Uberblick uber die Pflegeplanung nach dem Pflegeprozess sowie eine Muster-Pflegeplanung. Besonders praxisrelevant: Zentrale Pflegedokumentation, Aktivierungsnachweis, Hinweise auf Begrundungen und Individualisierungen sowie Evaluationsdaten, Evaluationsbogen, Meldebogen (Zwischenfall), Sturzprotokoll, Schmerzerfassung, Quantitat und Qualifikationsprofil der Pflegeinterventionen. Neu in dieser 7. Auflage sind die Risikoassessments zu Thrombose, Pneumonie, Soor, Parotitis, Kontraktur, Zystitis, Harninkontinenz, Dehydratation und Essverhalten und die PUdA-Skala zur pflegerischen Uberwachung der Arzneimittelwirkung.
